I have been trying to recover my old account. I have two. One I originally pre-purchased before the launch of Guild Wars 2, and another I purchased when PoF launched to start fresh and anew. I wanted to get back into my original account and use it as my main for the upcoming expac. The characters on that account are sentimental to me.
It is possible but have to go through many security steps such as showing your ID, showing game-cards (if you still have these) etc. I submitted a ticket to support and gave them as much info as I could - old account levels and names etc and they resolved it in about a week. EDIT: Finally got my account unlocked on August 8th. Reason for locking it was "someone tried to login". That someone was me :D TLDR: Was greeted by "Account locked for security reasons, contact support" message over a week ago, the only response I've gotten was an automated "we have received your support ticket" email.

A new one will be sent to the e-mail address that is associated with your account. By default, that'll be the e-mail address making up your user name. As for recovering a character name (since you need to know one of them to sign in), you might want to mention that to support when filling out the form for getting your account name back.
Basically they will ask you for whatever information you can provide: A character on the account, the world your characters were on, the four numbers that come after your username, or even the credit card you purchased the game with. They will also ask you for your address (the one you would have listed when you created your account).
A few days ago, I was having major issues with completing the final story mission in the human warrior storyline. It kept consistently kicking me from the game and, to date, I have not been able to complete it. I went ahead and went through troubleshooting it, all the way to the point of using a VPN (as someone else suggested).
